Dr. Moore has worked with the LGBTQIA+ Community for more than ten years through volunteer work, professional organizations, and community outreach. No matter your gender identity, your sexual orientation, or your relationship type, you are welcome at Nexus.

How does it work?

Our practice does not take traditional insurance, and there is never a copay. Instead, you pay us a monthly subscription fee, and you become a patient in our practice. This fee pays our team and makes sure that we can keep our practice small and personalized. Your health insurance plan still covers everything else it normally does, like prescriptions and lab tests, specialists, hospitalizations, etc.

You can call the office number or message through your patient portal whenever you have a problem or question. We’ll answer as fast as we can* (within one business day for routine care, sooner for urgent needs), and make sure you get the care you need.

If you need a visit with a specialist, we’ll help you arrange that! We’ll make sure you get a timely appointment, and then follow up with you to make sure the specialist’s plan of care fits within your overall health plan. You’ll use your insurance to pay the cost of the specialist visit.

*Even we sleep sometimes!

A physician who cares

After 2 decades working in patient care, for the insurance industry, and in medical business development, the one thing Dr. Moore knows is that he wants to do things differently! 

He wants to get to know you and your health care goals and needs. You’ll have a doctor who puts your individual health front and center. 

He will take time to understand your symptoms, answer all the questions you have, and determine diagnostics and next steps.

Also, by intentionally creating a small practice, he is focused on making sure his patients have quick access to high-quality, personalized care.

Jay Moore, MD (he/him) is board certified in internal medicine, and keeps up on all the latest clinical information in order to provide evidence-based medicine. We believe in science!

We Love Science

In fact, the logo for our practice is the image that’s on the Golden Record that’s engraved on Voyager 1, a satellite that was launched in 1977 (when Dr. Moore was 3 years old, yikes), and is still going strong.

The logo is a map of the universe that explains to any alien life forms that might find it where it originated. Having seen more than a few alien movies, we’re not sure that’s a great idea… but it’s still really cool!
