Hi there, my name is Jay Moore. I'm a board-certified internal medicine physician with 25 years of experience, and I'm the doctor at Nexus Primary Care. Nexus Primary Care is a new kind of primary care practice where you are at the center of all of the decisions that are made about your health. We concentrate on your needs, your health decisions, and the factors that will make you live the best life that you possibly can.
We're able to do this by keeping the practice very small, and what that means is that I, as the physician, have more time to spend with you. Instead of jumping through hoops at a normal primary care doctor's office, you can get into Nexus Primary Care whenever you'd like.
The way that we make that happen is by not taking insurance. Instead, you pay a fee to the practice which establishes you as a member. Once you're a member, we'll bring you in for an initial visit and spend about an hour talking to one another, getting to know each other and establishing your health goals. We can make a lot of progress in that first visit and make sure that you're getting the best care that you possibly can.
After the first visit, if there are additional things that you need or something comes up as an emergency or an urgent situation, you can get in touch with me any way you'd like. Maybe that means you send a text message, maybe you go to our secure web portal, or maybe you just pick up the phone and call the office. We'll get back in touch with you very quickly and figure out the best way to talk with you.
Maybe you need to come into the office and be seen, but many things can be taken care of over the phone or even by text message. Once you and I have established a plan of care, I'll call in your prescriptions, order any labs that you need, or just answer the questions that you've got.
It's a model that's unique and something that really takes you and your personal health goals into account.
