Medical records:

  • Need your medical record sent to another provider? Just give us a call at main number at (314) 464-9333 and we can send it out for you.

  • Need to have your medical records sent to Nexus? Download this Records Release PDF, fill it out, and submit it to the provider to authorize them to send your medical records to Nexus.

To ask a medical question:

  • Call or text Dr. Jay’s number, and he will answer these between 8 am and 8 pm. (This number is on your Welcome Letter; but if you need it again, please reach out.)

  • Call or text the main number at (314) 464-9333

  • Send a message through the patient portal

Communication Policies

  • The office phone is (314) 464-9333 and may be called anytime. Calls are returned the same day, usually within a few hours, as schedule allows.

  • After hours concerns: Active patients can call or text Jay’s cell phone. He answers calls from 8 am to 8 pm daily. If your concern is not during these times, he will respond the next morning. Otherwise, Just call the office number and leave a voicemail message. Please don’t call at night for non-urgent concerns.

  • Life-threatening emergencies: Call 911 and go to the emergency department.

  • Urgent medical needs: If you are unable to reach the doctor, please go to the nearest urgent care center.

Confidentiality

This Practice abides by federal privacy regulations and keeps your protected health information confidential. Your health information may be disclosed to plan your treatment or in determining eligibility for coverage or claims processing. You have a right to review and receive a copy of the complete Notice of Privacy Practices which outlines the full policy.
